Toronto Central LHIN Issues Calls for Proposals
The Toronto Central LHIN is issuing a Call for Proposals for 2010/2011 funding for the following services:
- Transitional Care Program (pdf)
- Enhanced Day Program for Adults with ABI (pdf)
- Concurrent Disorders and Problematic Substance Use Crisis Response (pdf)
- Community Outreach Workers/Peer Program (pdf)
Process
For any of the above calls, please complete a Health System Improvement Pre-Proposal (H-SIP). Please note that All sections of the H-SIP Form must be completed in order for the application to be considered. Proposals must be submitted to the torontocentral@lhins.on.ca by 5pm on Tuesday, June 15, 2010.
