Expression of Interest: Toronto Central LHIN’s Citizens’ Panel


The Toronto Central Local Health Integration Network is refreshing our Citizens’ Panel which was created to ensure that all Toronto Central LHIN strategies and initiatives are grounded in and guided by what matters most to the individuals, families and communities we serve. 

We are looking to establish two separate citizen advisory groups: the Citizen’s Panel and a new Home and Community Care Patient and Family Advisory Council (PFAC). While each will serve as forums for the Toronto Central LHIN to engage with the patients, caregivers, families and residents of our health care system, the two groups will have unique focal points:

  1. Citizen’s Panel this committee will provide insight on strategy of our population health planning. It will collaborate on patient experience initiatives, help design effective patient engagement, and give input into LHIN policy and standards that guide LHIN initiatives. Deadline to apply for membership to the Citizen’s Panel is Tuesday, October 10th, 2017

     

  2. Home and Community Care PFAC this committee will provide input on how we organize and deliver home care services. It will inform and advise the Toronto Central LHIN Home Community and Care Division on quality improvement initiatives, policies and procedures and key initiatives such as the caregiver strategy. Deadline to apply for membership to the Home and Community Care PFAC is Friday October 20th, 2017

All panel members will be provided with orientation to support meaningful participation. Members of both the Panel and the PFAC are eligible for reimbursement of costs related to participation in these committees. Members for each group can serve terms of up to two years with the possibility of renewal, and can step down at any time.

Membership is extended to all Toronto Central LHIN residents or residents served by the Toronto Central LHIN. However, any resident working for or on the Board of a LHIN-funded health service provider or another health care agency will not be considered due to potential conflict(s) of interest.

Recruitment will continue on an ongoing basis in order to maintain a group of interested individuals that serve as advisors to the Toronto Central LHIN through other means (focus groups, project level committees, surveys etc.), and to facilitate each group’s rotating membership. We will be interviewing shortlisted candidates for the Citizen’s Panel in mid-October and interviewing for the Home and Community Care PFAC in early-November.
