Integrated Health Service Plan 2013-16

In three years the TC LHIN aims to have a more integrated health care system that responds to patients' diverse needs and provides for a better experience as people move across the system. TC LHIN's third Integrated Health Service Plan (IHSP-3) builds on the Strategic Plan and sets the course over these next three years.

The TC LHIN's Strategic Plan sets out the steps that will be taken over the next two years to improve the quality of care for all, achieve better outcomes for patients, and put the local health system on a more sustainable footing.

We would like to recognize the community members and stakeholders who took the time to provide us with feedback on the Plan.  The Strategic Plan is based on relevant data and evidence and reflects a range of perspectives about how to transform the local health care system for patients today and in the future.

Summary of Stakeholder Engagement

In winter 2012 the Toronto Central LHIN began the process of developing a new Strategic Plan to guide local health care system through a challenging and significant period of change.

The overall Aim of the TC LHIN’s Plan is: Transform the system to achieve better health outcomes for people now and in the future.

In developing the plan the TC LHIN assessed relevant data and evidence about health care system performance, population health needs and gaps, and the experiences in health care systems in Ontario and other jurisdictions.

And we asked a range of stakeholders, patients and community members for their feedback on our Plan. 

Through this process we gained a variety of thoughtful perspectives, insights and ideas for change.  Some of the feedback has been incorporated into the final document.  The bulk of what we heard will be incorporated in some way into the design of and the implementation plans for the initiatives.
