MSK Rapid Access Clinics

The Toronto Central Local Health Integration Network (LHIN) is committed to improving the quality, access and appropriateness of care for people with musculoskeletal (MSK) conditions. This initiative includes the implementation of a new model of care across the Toronto Central LHIN referred to as Rapid Access Clinics (RACs).

Rapid Access Clinics for musculoskeletal conditions provide patients with timely access to comprehensive assessments, education about their condition, and self-management support tools. Patients also receive recommendations for appropriate advanced interventions and referrals for surgical consultation where necessary.

Rapid Access Clinics support primary care providers in managing patients presenting with MSK conditions, providing one point of contact through the MSK Central Intake Office for referrals into standardized and proven clinical pathways.

Through this shared-care model, the clinicians will work with primary care providers to keep them informed of the recommended course of treatment for managing each of their patients’ care out of the Rapid Access Clinics.

In addition to providing patients with timely access to the appropriate high quality care, Rapid Access Clinics will also support specialists to receive more appropriate referrals, which allow for more predictable practice and improved wait list management, helping to support a more sustainable health care system.

Referring Patients to MSK Rapid Access Clinics

Using a standardized referral form, primary care providers send referrals to the Musculoskeletal (MSK) Central Intake Office, which provides criteria based triage decisions to facilitate patient care.

The referral form is then routed to one of the several practice locations, located in communities and hospitals across the Toronto Central LHIN.

Within four weeks of referral, a regulated health professional provides a thorough standardized assessment. Patients requiring more advanced assessment and management are identified, and referred on to other providers within this pathway.

Over time, pathways for all musculoskeletal conditions will be integrated into these regional musculoskeletal intake, assessment and management models.

MSK Central Intake Office

The Toronto Central LHIN’s Musculoskeletal (MSK) Central Intake Office is the central point of contact for primary care providers to refer patients to the Rapid Access Clinics (RACs) and access services for their patients.

The MSK Central Intake model provides standardization of information requirements (completeness criteria), +/- imaging requirements for referral, as well as tracking, measurement, and reporting of wait times for patients.
