BUILDING ON THE FIRST PLAN

An evolving strategy that began with the first IHSP

The first Integrated Health Service Plan created a foundation for change by building a solid understanding of the local health system and its communities and health service providers. In the 2010 - 2013 Integrated Health Service Plan, the Toronto Central LHIN focuses on accelerating the process of transformation to achieve:

  • A better organized local health care system that is primed to put increasingly intensive efforts into prevention, promoting a healthier population, and empowering people to be full participants in their health care.
  • Better organized services for people with the highest needs, to build a health care system that performs more effectively for everyone. This will require health service providers to increasingly collaborate across disciplines and sectors – and will foster a culture where creating maximum value for the consumer is the shared mission of all health service providers, not just the domain of individual organizations.
