With its Resource Matching and Referral project, the Toronto Central LHIN is using technology to streamline referrals and ensure patients and clients get the right service at the right time.

A shared vision for a health system all Ontarians can count on

The Toronto Central LHIN's long-term vision is shared by all 14 LHINs and embodies a health care system that all Ontarians can count on regardless of their individual circumstances and where they live. In the Toronto Central LHIN, this vision is crystallized in the following statement:

A health care system that helps people stay healthy, delivers good care when people need it, and will be there for our children and grandchildren.

What does this vision mean for the people in Toronto Central LHIN?

The Toronto Central LHIN envisions long-term vision sees an equitable and value-rich system that makes the best use of health care dollars. Regardless of race, income, age, education, sexual orientation and language, every person in the Toronto Central LHIN will have timely access to the high-quality health care options they require, leading to more equitable health outcomes. At the same time, those with the most serious and complex conditions will receive the extra support they need.

In this health system of the future, services will be organized and delivered in a way that achieves the best health outcomes while keeping down costs. People will be able to evaluate the value of local health care services because the LHIN and health service providers will measure and report on the outcomes achieved for dollars spent.
