Toronto Central LHIN

No one organization alone can deliver all the services that people need.  The  Local Health Integration Networks bring local health services together around one common set of goals and one plan for patients and communities.  

LHINs involve local health providers and community members in setting priorities and solving problems so local health programs reflect what’s important to communities.  Click on the Better Health Care, Healthier Communities button for highlights about how health care is changing in the Toronto Central LHIN.

Value of LHINs

The Toronto Central Local Health Integration Network (TC LHIN) is responsible for planning, coordinating and funding key health care services in central Toronto for approximately 1.15 million people. 

We fund 174 health service providers including hospitals, the Toronto Central Community Care Access Centre, community support services, Community Health Centres, mental health and addictions agencies and long-term care homes. In addition to serving our local community, health service providers in the TC LHIN provide specialized health services to a large number of Ontarians who live in other LHINs.

Senior Friendly HospitalSenior Friendly Hospital Care Across Ontario

Ontario’s Local Health Integration Networks (LHINs) are leading an initiative to improve seniors’ health and prevent their physical and mental decline in hospital. 

As part of the first phase of the strategy, all 155 adult hospitals in Ontario have assessed their successful seniors’ initiatives, and identified opportunities for improvement, the results of which are outlined in a new provincial summary report.

Home First – Supporting Seniors' Health and Independence

Providing safe and timely care for seniors helps seniors return home once their hospital care is complete.  A partnership between the Toronto Central Community Care Access Centre, hospitals and community agencies, Home First is helping patients safely return home from hospital and long-term care homes.
